1. Quotes are onli valid for 15 days.

2. If you make a deposit of $500.00 usd, your quote will be valid for 6 months from the payment moment.

3. To book a surgery date, you must pay the deposit described before.

​

4. Deposits are NO REFUNDABLES AND NO TRANSFERABLES in any situation, unless the cancelation was due to a major cause of the surgeon.

5. If you book a date, and for any reason you have to reschedule, it would be with NO penalty up to 15 days before the surgery. If you wish to reschedule in the last 15 days before the surgery, you will be penalized with the loss of the deposit. This would no apply if the rescheduling  must be made due to an emergency situation, a medical condition that does not allow the surgery to be performed at that time, or any other situation that is not under your control. In that case the surgery can be rescheduled without panalty, showing a valid proof of the special situation.

6. If you don't book your surgery in the 6 months that is your quote valid, you will loss your deposit. And in order to rebook, you will need a new quote, with a price adjustment if apply, and a new deposit.

7. Please read the Presurgical List and the Day of Surgery Cancelation policies to be in full knowment and understanding of them, before putting a deposit.

9. Please download the Terms and Conditions Sheet, sign it and send it to us with your deposit.
